For your family’s convenience and wellness, BT will host a flu shot clinic for BT students at all grade levels, administered by Kohll’s Rx, on Tuesday, October 8 during the school day.
Lower School students will be escorted from their classroom by staff to receive their shots. Middle and Upper School students may be excused during class or visit the clinic during Advisory, flex, or study hall to receive their shots.
To get your child(ren) a flu shot, you must complete these three steps:
- Sign-up your child(ren) by Tuesday, October 1.
- Print and complete a consent form for your child(ren). A form must be completed for EACH student receiving a flu shot.
- Send the following with your child(ren) to school for their appointment:
- Copy of signed consent form(s).
- Photocopy of parent/guardian’s driver’s license, state issued ID card, green card, or passport. ID must match the signature on the consent form.
- Photocopy of parent/guardian’s health insurance ID card/prescription drug plan card with the Rx BIN/PCN/GRP codes.

Please Note:
- Kohll's Rx will file insurance claims on behalf of all clinic participants.
- Students age 18 and under who are covered by Medicaid must receive flu vaccinations from their primary care physician and are not eligible to participate in this clinic.
- COVID vaccines will not be available at this clinic.
